It is a safe, affordable platform, it has a simple interface that facilitates its use; through its efficient dashboard, it allows me to fill out forms of my clinical history immediately, I also like its tools to view my appointments for the month to speed up each process doctor simply and safely.

My experience with FormDr is really good.

I can access my medical history easily. I can view my appointments from anywhere, safely.

I can book dental appointments or consultations online, fill out forms and send documents securely.

The forms' flexibility to be customized was superb, and new features were constantly added. It is possible to adapt the state according to the needs of the customer using several free templates that are extremely simple to use.

I also like its tools for seeing my monthly appointments, which makes it easier and safer for doctors to complete each procedure.

The customer service was utterly inaccessible the last time I utilized FormDr for one of our clients.

It can be challenging to develop templates that work for us.

The customization ability of the forms was excellent and they continuously add more features. The former support team was reachable primarily through email and they often responded within one business day and sooner for emergencies.

There was a slight learning curve in making those forms on our own. Sometimes the site freezes up and you can't finish what you are working on.

It helps me access my medical report easily and even I can view and make my appointments from anywhere safely. Helps for a better survey regarding doctors.

It reduces the hassle of sending documents by forwarding the links for digital signature. The templates are great. One can edit them and manage them per their need to some extent.

The site issues are one to be worried about as most data gets lost and the screen gets stuck. For exceptional cases, the templates can't be very accommodating.

Mostly using it to get the signatures digitally. It saves a lot of time and follow up which happens in case of normal documents.

It is quite simple to use, and we offer a variety of templates to choose from. We have reviewed our existing schedule.

Customer support is not good. Develp templates to our convenient is bit difficult.

Last I used FormDr for one our clients and the customer service was unreachable through any means.

I havent used it for a long time but hopefully the CS would have improved.

We used FormDr for one of our clients to send surveys to patients and get their review on the process of the overall experience.

We noticed that people could also book online appointments in advance which eliminated the last moment hassle.

Following things I like the most about FormDr:

1. Multiple free templates are available, which are very easy to use.

2. Form can be customized as per the need of the Customer(doctor).

Following things I do not like FormDr:

1. No live customer support is available.

2. Dashboard is not user friendly.

3. Pricing is high.

It makes the handling of a database of patients easy. So life becomes easy and less hectic.

How simple and user-friendly the forms are to send to patients and receive back.

It does not integrate with our patient software system to automatically upload into their chart.

They were helping us solve the issue of paper froms for new patient information and office and clinical treatment consents.

I like that this provides a solution for patients to fill out medical information, intake forms, etc. prior to coming into the office. It saves time and is secure.

It takes some effort to develop customized templates but once developed use is very easy. More default templates with steamlined customization would be ideal.

This adds to the efficiency of our office and improves the patient experience. We cut down on wait times and overal appointments time for the patients when we have pertinent forms completed in advance.
